About Cook Children's Health Plan
We are a local, non-profit health plan that cares about our community. Our plan provides health coverage for CHIP, CHIP Perinatal, STAR (Medicaid) and STAR Kids Members in the Tarrant county service area. The counties we serve includes Tarrant, Johnson, Denton, Parker, Hood and Wise. Our health plan allows you to choose from many doctors, clinics, hospitals and specialty services. Our network providers care for all ages, not just children.

To follow the Center for Medicare and Medicaid Services (CMS) rules, Cook Children’s Health Plan (CCHP) must post information each year about the medical items and services that require prior authorization, except for drugs. Prior authorization is a request from your doctor for services you may need. The health plan’s trained staff review the information sent from your doctor to decide if these services can be approved. This report includes all decisions we made last year, such as approvals and denials. CMS requires CCHP to share this information on our website to help our community understand how our prior authorization process works.
