Check eligibility
View your or your child's current and past eligibility and coverage with Cook Children's Health Plan, and see referral information.
Check authorization status
View your or your child's current and past authorizations submitted by your provider.
Check claim status
View current and past claims for services that you or your child received from your provider.
Member ID card
View and/or print a copy of your or your child's Member ID Card. You can also request a replacement ID card. Digital ID card for Apple Wallet available now!
Find a doctor
Find a doctor or update your primary care physician in the network with Cook Children's Health Plan.
