Behavioral Health Benefits

Members of Cook Children’s Health Plan could receive Behavioral Health rewards too. Cook Children’s Health Plan is concerned about the behavioral and mental health of our Members. We promote whole person care and understand the importance of an individual’s physical and mental health. Cook Children’s Health Plan want to reward you for taking care of the “whole person”.

Some of the rewards you can receive are:

• $25 gift card for Members 13 years of age and older who have a diagnosis of Alcohol or Drug Abuse (AOD) or dependency and receive treatment through an inpatient AOD admission, outpatient visits, intensive outpatient encounter or partial hospitalization, within 14 days of diagnosis.
• One-time $25 incentive through a reloadable prepaid card to Members ages 6 to 12 newly diagnosed with ADHD and dispensed an ADHD medication for the first time who follow-up with a practitioner with prescribing authority within 30 days of receiving the medication.
• $25 gift card for members who complete a follow-up visit with their behavioral health provider within 7 days of leaving the behavioral health facility.
• Online social services resource directory to locate community supports such as food and nutrition, housing, education, and employment services

Disease Management

Our Disease Management Program could help you and you can earn rewards. You could earn rewards valued at $350 to help and manage your conditions.

For Asthma, those rewards are:

• $100 towards a Hepafilter Vaccum
• $100 towards products to assist with Asthma
• $50 Pest Control Services

For Diabetes, those rewards are:

• Earn up $150 per year to taking care of your diabetes (only STAR members)
• Access to the Boys and Girls club at no cost where available to all qualifying Members.
