Diabetes Management

$150 Total Gift Card Reward per year for Members ages 18 to 20 with a diagnosis of diabetes who complete the following ($25 gift card per component per year, $150 maximum).

• Diabetes Health Risk Assessment
• Having Hemoglobin A1c tested
• Hemoglobin A1c <8%
• Both eGFR (estimated glomerular filtration ratio) and uACR testing (urine albumin-creatinine ratio)
• Maintaining blood pressure of <140/90
• Retinal eye exam.

Reimbursement for Pest Control Services

Diabetes Prevention Class

The diabetes Prevention Program is a full year program for Members age 18 and up with trained coach to learn and maintain healthy lifestyle through cooking lessons, exercise, and nutrition seminars.

Dental Services

Up to $350 for basic and major dental services for pregnant women ages 21 years and older who are enrolled in the Cook Children's Health Plan Baby Steps program.

• Must be enrolled in Pregnancy Medicaid and Baby Steps Program.

Lifestyle Change Program

16 week program for child and caregiver to address childhood obesity. The program focuses on healthy eating, regular physical activity and behavior change to empower children to live a healthier and active lifestyle.
