24/7 Nurse Advice Line

Registered nurses are available 24 hours a day, 7 days a week to offer you support when you are not feeling well and help you understand what to do next.

Non Emergency Medical Transportation

• One round trip per month. Rides can be to classes, WIC offices, Member events, grocery stores, job interviews, and other types of assistance.
• Additional riders if the Member/LAR cannot make other arrangements.

Reimbursement for Pest Control Services

$50 per year for pest control services for Members with a chronic respiratory condition enrolled in the asthma disease management program and/or in service coordination

Diabetes Prevention Class

The diabetes Prevention Program is a full year program for Members age 18 and up with trained coach to learn and maintain healthy lifestyle through cooking lessons, exercise, and nutrition seminars.

Extra Vision Services

$125 value for eyeglass frames/lenses or $100 for contact lenses/fitting fees per year.

Help for Pregnancy/Parenting Classes

Up to $100 for prepared childbirth, breast-feeding, or parenting classes per pregnancy. May be on-line or in person.

Prenatal Visits

• $25 gift card for pregnant women for attending their first prenatal visit within 1st trimester or 42 days of enrollment.

• $25 gift card for pregnant women for attending a visit within the 6th month of their pregnancy.
• $25 gift card for pregnant women for attending a visit within the 9th month of their pregnancy.

• A $50 gift card for postpartum members who complete a postpartum visit with an approved provider between 7-84 days after delivery.

Car Seat Class

Car seat class for mothers of Members under the age of 1 year. A free car seat will be given after completion of the class.

Safe Baby Sleep Class

Safe Baby Sleep class for Parents/Guardian of Member under age 1 year. A pack-n-play will be distributed for those who attend and complete the class.

Food for Pregnant Women

$50 of food for pregnant women. They must complete the health screening within 30 days of becoming a Member.

Well Child Checkup Gift Card Rewards

• $25 gift card for members age 2 to 19 years who complete a Well Child visit per year.
• $25 gift card for members completing a checkup at 2 weeks of age.
• $25 gift card for members completing a checkup at 2 months of age.
• $25 gift card for members completing a checkup at 4 months of age.

GED Reward

One-time $25 gift card for Members 17 years and over who enroll and complete a GED class.

Water Safety Course

Virtual drowning prevention course for parents/guardians of Members ages 0-4 will receive a water watcher tag and educational brochure.

Tobacco Cessation Gift Card Reward

Blood Pressure Self Monitoring Program

Four month program that includes personalized consultations and nutrition seminars with a Healthy Heart Ambassador.

Lifestyle Change Program

16 week program for child and caregiver to address childhood obesity. The program focuses on healthy eating, regular physical activity and behavior change to empower children to live a healthier and active lifestyle.

Member must be between 7 to 13 years of age.

Extra Help for Members Diagnosed with Asthma

• Up to $100 per year for Members with an asthma diagnosis and enrolled in the Asthma Disease Management Program for:
• Pillow covers/mattress covers
• Vent cleaning
• Deep carpet cleaning
• One peak flow meter and holding chamber or spacer.

Temporary Phone Help

Reimbursement for up to $25 per quarter for additional data for Members who qualify for the federal lifeline program.

Extra Help for Members Diagnosed with Alcohol or Drug Abuse (AOD)

$50 gift card for Members 13 years of age and older who have a diagnosis of Alcohol or Drug Abuse (AOD) or dependency and receive treatment through an inpatient AOD admission, outpatient visits, intensive outpatient encounter or partial hospitalization, within 14 days of diagnosis.

Behavioral Health Follow-up Visit Reward

$50 gift card for Members who complete a follow-up visit with their Behavioral Health Provider.

Extra Help for Members Diagnosed with ADHD

$50 gift card one-time for Members newly diagnosed with ADHD who follow up with a practitioner within 30 days after receiving the new medication.
